Physician Certification Statement (Ambulance)
Required for non-emergency ambulance transports. A licensed medical professional must certify that ambulance-level care is medically necessary for billing and reimbursement.
Physician Certification Statement (Medicar / Service Car)
Required for non-emergency Medicar or wheelchair transports. A licensed medical professional must certify that the patient cannot safely travel by standard transportation.
